2013-2014 Residency/ Enrollment Information
Proof of residency must be made at the beginning of each school year, change of address, and any time a new student enrolls.

Documents Requested:

• Parent/Legal Guardian Photo ID
• Legal Guardianship Paperwork - if applicable
• Mortgage or Lease Agreement
• A current gas or electric bill 30 days or less: if utilities included in Lease or no utilities in your name, please provide two more proof of documents.  (No shut off/final notices and No water/sewer bills be accepted)
• Car Title/Car Insurance Paper
• Court/State/Gov't Docs
• Life Insurance
• Student Loan Information
• Pay Check/Pay Stub
• Unemployment Verification
• Original Student's Proof of Birth upon enrollment
• Student Immunization Records

Form 2 - Families Living with a District Family

• Both families must be present for enrollment registration
• Family living in the district must provide three proofs of residency

(Take 61 st Street east from Blue Ridge Blvd to the stop sign, turn left, bldg on right).
