Form 2 Families
A Form-2 family is a family who lives within the household of another family whose residency aligns with the Raytown School District boundaries.
Form 2 - English Form 2 -Spanish
Both Families will be required to complete and sign the Form 2 Affidavit form.
District Family – Homeowner/Leaseholder will need to provide
- Government issued Photo ID
- Mortgage or Full Lease Agreement
- Electric, Gas or Water/Sewer bill 30 days old or less (No Shut-off/ Final Notice will be accepted). If utilities are included in lease, or no utilities in your name, please provide one additional document for approval from the list below.
Parent/Guardian Information - Documents required from family living with District Family:
- Parent/Legal Guardian government-issued photo ID/passport
- Student’s Proof of Birth document
- Student’s immunization record
- If not biological parent, provide Legal Court Guardianship Paperwork (No Power of Attorney accepted)
In Addition, you must provide one parent document, no more than 30 days old, from the list below to verify your name and current address:
- Bank Statement/Cash App/Chime
- Car Payment Information/Insurance Paper
- Court/State/Government documents
- Life Insurance
- Paycheck / Pay Stub / Unemployment Verification
- Student Loan Information
